In order to access subsidised Home Support services, your support needs will need to be assessed.
There are two types of assessments for Australian Government home support programs and packages;
- a basic assessment with a member of the Australian Government Regional Assessment Service (RAS) for entry level support at home
- a comprehensive assessment with a member of an Australian Government Aged Care Assessment Team (ACAT) for more complex support at home.

Southern Cross Care also provides a self-assessment form which will get you thinking about what your needs are, considering what daily activities you might need some extra help with.
Our trained and qualified staff will work with you, and your family, to identify your individual needs, goals and aspirations. We will then create a personalised care and support plan including the services you need to live better in your own home.
Part of this planning process is to identify when and how often you would like to receive services. This helps to ensure that our services are provided at times that are convenient for you, working in with your regular schedule.
You will be provided with a copy of your personal plan, so you can stay fully informed about your Home Support services.
