To access funding for any aged care package, an assessment of your situation and needs must be performed by the Aged Care Assessment Team (ACAT). This is the case whether you are looking to remain living at home, transition from a hospital stay, move into residential care, or are a carer in need of respite services.
What is the Aged Care Assessment Team (ACAT)?
The ACAT is generally made up of a range of health professionals with experience in aged care. The Australian Government engage State and Territory Governments to manage ACAT services.
Assessments by ACATs are funded by the Government, and do not cost you anything.
How do I contact an ACAT?
To find the ACAT closest to you, simply:
- Call 1800 200 422
- Use the ACAT finder on the MyAgedCare website or ask your doctor.
Southern Cross Care provide a self-assessment form which will get you thinking about what your needs are, considering what daily activities you might need some extra help with.
For us, it provides information so together we can develop a plan that will give you the very best level of assistance so you can live well and remain at home for as long as possible.
