Whether you're recovering from an injury, a hospital stay or facing challenges with daily activities, our restorative and transition care services can improve your health and wellbeing to help you continue to live independently and be Better for life.

What are restorative and transition care services?

 

Restorative and transition care is time limited, intensive, short term care that can help you recover your ability to do everyday tasks and remain living independently at home. It can be delivered in your home or in our Health & Fitness Centres and is split into two similar programs for different purposes.

The two restorative care programs offered by Southern Cross Care are Short-Term Restorative Care, for people who need specialised support to remain living independently, and the Community Transition Care Program, for people who need support to go back to living at home following a hospital stay.

Short-Term Restorative Care

The Short-Term Restorative Care program is an Australian Government initiative, designed to help older Australians stay independent and living in their own homes.

It is an eight week program of targeted physical activity and therapy services, delivered to clients in their own homes or local communities.

Participation in the Short-Term Restorative Care program is subsidised by the Australian Government. We can work with you to deliver high quality services in your own home, or at our world class Health & Fitness Centres.

Community Transition Care Program

The Community Transition Care Program (CTCP) provides a range of specialised services for up to 12 weeks to eligible older people who need support getting back on their feet following a hospital stay. 

The program aims to help individuals regain as much independence as possible and avoid returning to the hospital or the need for longer term care. Once you are home, our program aims to optimise your function, health and wellbeing with targeted allied health and supportive care services.

At Southern Cross Care, our restorative and transition care services are provided by a dedicated team of allied health professionals and therapists. We will work with you to create a personalised program tailored to your individual needs and goals.

 Our team will coordinate a range of tailored services designed to help you achieve your health goals. These may include:

  • Exercise classes to improve strength, fitness, balance, and mobility
  • Physiotherapy
  • Massage therapy
  • Podiatry 
  • Dietetics
  • Nursing
  • Social work

 We also offer assistance to help you reconnect with your local community through engaging social programs, supporting your overall wellbeing.

Eligibility

Eligibility for services

 

Short-Term Restorative Care (STRC) 

Southern Cross Care offers Short-Term Restorative Care to individuals aged 65+ in South Australia and Darwin. 

To be eligible, you must:

  • Be experiencing difficulty with day to day activities
  • Have not been hospitalised due to a significant incident in the previous three months
  • Not be receiving end of life care or a Home Care Package
  • Have not received Short-Term Restorative Care services within the past six months

 

Community Transition Care Program (CTCP) 

To be eligible you must:

  • Be in hospital and nearing the end of your hospital stay
  • Be able to benefit from a program that can help improve recovery and restore independence as much as possible

Fees and Funding

Understanding costs for care services

 

Government Subsidy

Restorative and transition care services are subsidised by the government, making them more affordable. You will only need to contribute to the cost if you are financially able to do so.

 

Cost Variation

The amount you may be asked to pay depends on:

  • The type of services you access.
  • The level of care required.

There are strict rules in place to ensure charges remain fair and reasonable.

After Restorative Care

Access affordable Health & Fitness services 

 

After completing the Short-Term Restorative Care or the Community Transition Care Program you can continue to access a range of subsidised services with Southern Cross Care to remain independent. 

These three options help you to access our Health & Fitness services to stay healthy with low out of pocket costs;*

 

Private Health Insurance 

Depending on your coverage, your private health insurance could pay for part or the entirety of allied health services like Physiotherapy, Exercise Physiology, Massage Therapy, Podiatry and more. 

 

Medicare funded GP Management Plan 

Available to anyone with a chronic medical condition via a GP referral, you can receive five subsidised allied health sessions per year (including Physiotherapy, Exercise Physiology, Dietetics, Podiatry) 

 

My Aged Care funding 

See if you’re eligible to receive services at our Health & Fitness Centres or in your own home through the Government’s Commonwealth Home Support Programme or a Home Care Package

FAQs

The Community Transition Care Program (CTCP) provides similar services but specifically to people who have had a stay in hospital and need intensive support for a period of time in order to return to living safely and comfortably at home. The program aims to help individuals regain as much independence as possible and avoid returning to the hospital or the need for longer term care. Once you are home, our program aims to optimise your function, health and wellbeing with targeted allied health and supportive care services.

To start the program you need to have an assessment with My Aged Care. Once My Aged Care has assessed you as appropriate for either STRC or CTCP please get in contact with Southern Cross Care on 1800 852 772. If you require further information in regards to accessing a My Aged Care assessment, please contact My Aged Care on 1800 200 422.

To be eligible for the Short-Term Restorative Care program, you must be: 

  • Slowing down mentally, physically or both 
  • Aged 65+ or 50+ for Aboriginal and Torres Strait Islander people 
  • Not receiving a Home Care Package 
  • Not receiving services through a Transition Care Program 
  • Living in your own home 

You could be eligible for the Community Transition Care Program if you are: 

  • In hospital and nearing the end of your hospital stay 
  • Able to benefit from a program that can help improve recovery and restore independence as much as possible 
  • Assessed as eligible by an aged care assessor

The Short-Term Restorative Care program spans eight weeks, or 56 days. It can be accessed twice in a 12 month period. 

The Community Transition Care Program is 12 weeks long, and in some cases can be extended for a further six weeks if needed. 

You can access both the Community Transition Care Program (CTCP) and Short-Term Restorative Care (STRC) if you are eligible, but you must wait six months after completing the CTCP before starting STRC.

Our restorative and transition care services offer a holistic approach to improve your physical, social and mental wellbeing. 

You will identify a set of goals you want to work towards through the program and our team will help you to choose services that match your goals and needs.

  • Massage therapy 
  • Dietetics Group exercise classes 
  • Social work 
  • Nursing 
  • Personal care Aids and equipment 

Services may be delivered in our Health & Fitness Centres or in your home.

Restorative and transition care programs are subsidised by the government. Fees differ depending on the types of services you access but you will only be asked to contribute to the cost of services if you can afford to.
