Meet Jackson: Melbourne's Dedicated Community Nurse

Meet Jackson, the community nurse dedicated to helping clients across Melbourne with their health.

Jackson Chen works in the Southern Cross Care Home Care Melbourne team and said the Registered Nurse role fulfills his desire to contribute to society and help the community.

“I have always aspired to contribute to the community,” Jackson said. 

“During my Bachelor of Nursing education I discovered the unique role of a home care nurse, including the engagement of face-to-face communication, and performing hands-on tasks in the client's home – creating a diverse and fulfilling everyday experience.”

Jackson’s role as a community nurse is a special one, as many Home Care providers do not have a dedicated Registered Nurse, they instead rely on a nurse who also acts as a coordinator or simply hire agency nurses.

Having Jackson as the nurse supporting all Home Care clients in Melbourne means the team can deliver a higher quality of care.

It also means clients experience continuity of care and don’t need to repeat their story or explain their health conditions over and over again.

As Jackson said, “I can promptly report any issues to the team, seek assistance, and provide consultations in a timely manner to deliver high-quality wound care and welfare monitoring. Our approach has contributed to significant improvement in clients with challenging wounds, and the regular welfare monitoring and education have enhanced quality of life for many of our clients.”

Jackson takes pride in the fact that he can provide education to clients, so that they build their skills and independence and can live at home for longer.

Building rapport and strong relationships with clients and their family members, Jackson said, is the best thing about his job.

“It is also very rewarding to see that many of my clients actually look forward to my visits weekly, they will then share their news and updates to me for that week. I feel that my work is appreciated and that I am needed,” he said.

One 95-year-old man, who was receiving home care services as part of palliative care, had developed pressure injuries due to his lack of mobility.

Jackson stepped in to look after his wounds and provide education on how he could improve his mobility and quality of life.

“He enjoyed our chats, always focusing on the positive aspects of life,” Jackson explained. 

“His quality of life in the last few months was well maintained.

“He and his wife even shared their beautiful Mediterranean desserts with me to bring back to the office and share with other staff. 

“His wife, witnessing the excellent care we provided, eventually became one of our clients as well.”
